Wasn't this the DC-10 out of Chicago, the engine "fell" off the wing during take-off rotation.

The number one engine fell off, ripping out hydraulics which kept the leading edge slats deployed. When the L.E. slats retracted, the airplane went into an uncontrollable roll to the left, became inverted, and hit the ground moments after liftoff from runway 32R at O'Hare.

They tried to do an engine swap using a fork lift and broke the engine mount. This is what happens when people who don't know what they are doing get a brain fart. People die.
